Pelvic Organ Prolapse Continuing Education Seminar – June 2018

This Boost Camp one day intensive seminar is designed for pelvic floor physical therapy specialists. It will teach you advanced techniques expanding your repertoire of prolapse exam and treatment progression. You are invited to submit a case study for course review.

The seminar includes a pre- course home study module that will provide resources to help understand the anatomy (from the vocal cords and glottal structures to the pelvic floor facial support) and symptom manifestation of pelvic organ prolapse (POP). It also has an extensive review of pessary guidelines and medical diagnostic and assessment techniques.  Review of this information will allow the one day seminar to focus on examination techniques and treatment.

Kathe's considerations about lifestyle interventions and prolapse go far beyond the specific teachings of prolapse exam and exercise. She incorporates the brain connection into the learning process. The brain is an organ that explores the internal and external environment. It allows responses and behaviors to change based on the sensory input it is given. Kathe brings in strategies of functional tasks with breath and appropriate muscle activation to help the prolapse patient maintain change and self-awareness.Gail Wetzler, DPT, PT, EDO, BI-D

The morning labs will include internal vaginal examination techniques for anterior, posterior and apical support changes, including assessment for levator ani avulsion and measurement of external perineal measurements used for prolapse quantification. You will learn appropriate examination instruction and the difference of summarizing your findings for the patient and for your medical records.

Afternoon lab time will focus on functional evaluation and treatment.  You will learn management of intra- abdominal pressure with attention to postures and the glottis, breathing and pelvic diaphragms. We will review the current research on the benefits of lifestyle intervention and pelvic floor exercise that will help you formulate the best comprehensive rehab approach that empowers the women to be active.    Some of these techniques can also apply to your patients with incontinence and types of pelvic pain.

Prerequisites: Prior training in internal pelvic floor examination. Prerequisite assignments (home study module) issued upon registration.  Independent review and study is required to prepare for the boost camp seminar. Content is not intended for use outside the scope of the learner's license or regulation.

Teaching Level: Intermediate to advanced physical therapists and PTA's. Pregnant women should have a release letter from their primary obstetrics provider for internal evaluation participation. Participants will practice exam techniques on one another.

Objectives: This has two components, home study and live seminar.

Home study module includes articles, guideline reviews, website links and on-line videos (included with course registration). This independent preparation for the course could apply to your state CEU requirement. i.e. WA state Continuing Competency Requirements.

Upon competition of the POP home study module the participant will be able to:

  1. List key patient history and  2 questionnaires that will identify sx and risk factors for POP.
  2. Name 3 key visceral fascia anatomy structures important for pelvic organ.
  3. Review the indications and use of 3 specific pessary types for POP.
  4. Describe 2 common findings with the diagnostic tests of POP-Q, MR defecography and levator ani avulsion.
  5. Describe 2 psychosocial patient perspectives on POP.
  6.  Complete a literature reviews (summaries and/or links provided) 
    • Review the POP literature (summaries and links provided) regarding Intra-abdominal Pressure (IAP).
    • Review POP surgery approaches and updates on the use of mesh.

Live seminar:  Upon completion the participant will be able to:

  1. Describe symptom presentations in 6 types of POP as they relate to fascial support.
  2. List the 3 modifiable conditions that could affect results of prolapse examination.
  3. Palpate with internal vaginal examination for levator ani avulsion, the ATLA and the ATFP
  4. Perform POP examinations both internal vaginal and external POP-Q measurements (gh and pb) using appropriate straining instructions.
  5. State 5 prolapse self-care/lifestyle intervention strategies for POP.
  6. Assess and teach breathing and IAP management in conjunction with therapeutic exercise and functional activity programs for POP.

Teaching how to manage IAP and return to active lifestyles. Case studies
